Best phrasing to ask a girl out?

I want to ask a girl out (I'm male if it's relevant). I want to be direct and make sure she absolutely knows it's not just friends hanging out. We're fairly candid around each other and I'm very confident, that's not the problem. I'm just unsure what exact phrasing I should use:

A) "Would you like to go on a date with me?"
B) "Would you like to go on a date with me to XXX on YYY?"
C) "Will you go out with me?"

I feel like B is better than A, but at the same time I'm so sure she's into me that C doesn't feel like a bad choice, even though it's not really asking her out on a date as such. I just don't want that limbo where you're going on dates but don't really acknowledge that you're a couple, and C avoid that very well. I do actually have a date planned by the way.

So, gals: if a fairly close friend you have a crush on was going to ask you out, which would you be most receptive to/would sound the best? Or do you have any other suggestions?
